I like [name_m]John[/name_m] with [name_m]Jack[/name_m] as a nn best here.
I would just go with [name_m]Jack[/name_m] if that’s what you intend on calling him. I prefer [name_m]Jack[/name_m] and [name_m]Wesley[/name_m] together too, and [name_m]Jack[/name_m] is a well established stand alone name now, so I don’t think it’s too informal.
I love both [name_m]John[/name_m] and [name_m]Jack[/name_m], probably my favorite boy name. I adore [name_m]John[/name_m] with nn [name_m]Jack[/name_m], but also [name_m]Jack[/name_m] on its own . In your situation, if you want to call him [name_m]Jack[/name_m] most of the time, I’d go with [name_m]Jack[/name_m] instead of [name_m]John[/name_m] because your older child doesn’t have a different name-nickname, (if that makes any sense). I mean, if your older would be [name_m]Henry[/name_m] nn [name_m]Harry[/name_m], or [name_m]Edward[/name_m] nn Ted I’d say [name_m]John[/name_m] nn [name_m]Jack[/name_m] would be a better option.

Personally I’d go with [name_m]Jack[/name_m], it’s generally considered an acceptable full name and there’s no situations I could think of where the name [name_m]John[/name_m] would present more opportunities than [name_m]Jack[/name_m]
If you’re going to call him [name_m]Jack[/name_m], I would put that on the birth certificate. It’s widespread enough as a standalone name to not come off as nicknamey.
I know a [name_m]John[/name_m] nn [name_m]Jack[/name_m] who doesn’t prefer either name over the other, so people usually call him Jackjohn.
