What's My Name?

I don’t know how to explain this well, but it goes something like:
"My first name is (classic, trendy, unusual, or whatever style you’re going for and other details like gender, popularity, theme, what letter it starts with, its’ origin, is the spelling traditional or different, etc…Ideas are endless here).
My middle name is (same steps as the first name. You may have as many middle names as you choose).
My surname begins with…(then maybe some other details. Hyphenated names are allowed).

What’s my name?"

Then the next poster replies with the name, saying “Your name is…”/“You are…” and posts their own thing.

You can phrase it in any way that works best for you, except you must say the last parts, both when you’re replying and asking.
